In my opinion, WICO is the only mod that significantly improves on the vanilla NPCs without making everyone look too pretty. HMB 2 would be my second choice; personally, I think that its "smoothing out" of the vanilla NPCs makes them lose some of the character their janky vanilla versions had.

I can't speak to all the versions, but the USSEP compatibility patch from the vanilla body version of WICO copies all of the ACBS sections of every NPC it touches from the USSEP. The only NPCs that are flagged Protected or Essential who are not that way in vanilla Skyrim are ones that the USSEP makes protected or essential.
